Dr. Juan José Hernández Herrera
01The specialty
The surgical specialist of the facial skeleton. After their base training, they complete years of hospital residency operating on the facial bones, the jaw joint and the tissues of the face and neck. Not a dentist doing aesthetics, nor a general practitioner with cosmetic courses: the surgeon trained specifically in the structure on which any facial result is built.
Certification by ACOMS — the American College of Maxillofacial Surgeons — verifies that training. It is a checkable credential, and one of the first things a patient should demand before facial surgery anywhere in the world.
Meet Dr. Hernández →The base that defines projection and proportion: chin, jaw, maxilla. Only surgery changes it.
The plane that supports the contour — like the bands of the neck — and that surface treatments cannot reach.
The visible surface. Its quality is maintained with complementary treatments; real laxity is corrected with surgery.
02 The core of the practice
Chin, double chin and neck form a single contour. Correcting them structurally is this practice's specialty.
Definitive projection and balance for your profile, working directly on the bone or with a custom implant.
Definitive removal of the fat under the chin and redefinition of the neck angle.
Deep correction of laxity and contour — skin, muscle and fat — with a long-lasting result.

04Traveling for surgery
Many of our procedures — and Costa Rica itself — draw patients from the United States and Canada. The clinic is in San José, about 30 minutes from Juan Santamaría International Airport (SJO), and your case is planned so one trip covers evaluation, surgery and the essential follow-up.
Write to us on WhatsApp in English. You share photos and medical history, and the team tells you whether your case is a candidate — before you book anything.
Evaluation, surgery and post-operative checks are scheduled for a single stay in San José. You fly home once the surgeon clears you.
Your recovery is monitored directly by Dr. Hernández, with remote check-ins after you return home.
05The specialist
Maxillofacial surgeon with more than 15 years of surgical practice, focused on facial contour and neck surgery. Board-certified by ACOMS, the American College of Maxillofacial Surgeons, which certifies specialists in this surgery.
His clinical judgment starts from a simple conviction: a natural result does not come from tightening the surface, but from correcting the structure that supports it.

07Maintenance
Botox, fillers and rejuvenation technologies are temporary therapies: they maintain skin quality, they do not correct structure. At Full Face they are indicated under surgical supervision — as a complement, never as a substitute for real correction.

Reference prices range from $500 (buccal fat removal) to $8,000 (jaw surgery) — you can see the starting price for each procedure on its page. The final cost is set at your evaluation, because it depends on factors that can only be determined by examining your case: the indicated technique, whether procedures are combined, the type of anesthesia and operating-room time. Write to us on WhatsApp — in English — and we will coordinate an evaluation and give you a real, itemized quote.
Yes. International patients start with a remote pre-assessment over WhatsApp, sharing photographs and medical history, and the in-person evaluation is scheduled for when you arrive in Costa Rica. The team tells you how many days you should plan to stay in San José depending on the procedure, so all necessary post-operative checks are done before you fly home.
A maxillofacial surgeon is the surgical specialist of the facial skeleton: after their base degree, they complete years of hospital residency operating on the facial bones, the jaw joint and the tissues of the face and neck. A dentist treats teeth and gums; a doctor with cosmetic courses applies surface treatments. For surgery that changes the structure of the face — chin, jaw, neck, nose — the maxillofacial surgeon is the specialist trained specifically for it.
Surgery corrects the structure — bone, muscle, deep fat — which is why its result is definitive or very long-lasting. Botox, fillers and technologies like Morpheus8 work on superficial tissues and their effect is temporary: months, not years. Both have their place: surgery solves the structural cause, and complementary treatments maintain skin quality. What matters is knowing which one your case needs — and that is defined at the evaluation, not from a catalog.
You don't feel the procedure itself: it is performed under anesthesia appropriate to the case. Post-operatively, the norm is swelling and discomfort manageable with common painkillers, not intense pain. Recovery depends on the procedure — from a few days for double chin surgery to one or two weeks for a neck lift — and you receive written instructions and direct follow-up from the surgeon. The clinic also has a hyperbaric chamber to accelerate healing when indicated.
The next step
At the first consultation Dr. Hernández studies your case, tells you which procedure is indicated — and which is not — and you leave with a clear plan and your quote. No pressure and no inflated promises. If you are abroad, it starts with a remote pre-assessment on WhatsApp.
